Bronchoplastic surgery. Results of treatment of patients with lung cancer

Summary. Aim. To evaluate the feasibility of bronchoplastic surgery on the lungs in cases of non-small cell lung cancer (NSCLC) and bronchial carcinoid tumours. Materials and methods. Results of treatment of two groups of patients are presented. In one group, patients underwent bronchoplastic lobectomy (group 1), in the other — pneumonectomy (group 2). The number of complications, overall and recurrence-free survival in both groups, as well as factors influencing results were analysed. Results. The number of postoperative complications in group 1 and group 2 was 12 (5.7%) and 8 (3.8%), respectively, depending on the presence of concomitant pathology. The median recurrence-free survival and overall survival in the groups did not differ significantly, being 24 and 48 months in group 1 and 23 and 44 months in group 2 (p=0.92 and p=0.98). Only the stage of the disease (p=0.016) and neoadjuvant chemotherapy (p=0.006) had a significant effect on the treatment results; preoperative radiotherapy was not used in either group. Conclusion. Bronchoplastic surgery is justified in cases of NSCLC and carcinoid tumours, requires implementation in specialised oncology centers, and necessitates careful monitoring and assessment of concomitant pathology as a significant risk factor.
