Validation of the Ukrainian version of the M.D. Anderson Dysphagia Inventory (mdadi) in surgically treated oropharyngeal cancer patients
Summary. Dysphagia is one of the most common complications of oropharyngeal cancer, significantly affecting patients’ quality of life. The M.D. Anderson Dysphagia Inventory (MDADI) is widely used internationally for dysphagia assessment in oncological patients, however, no validated Ukrainian version exists. Objective — to validate the Ukrainian version of the MDADI questionnaire in oropharyngeal cancer patients after surgical treatment. Materials and methods. This prospective observational validation study included 61 oropharyngeal cancer patients (mean age 57.0±10.3 years) who underwent surgical treatment. MDADI and EORTC QLQ-H&N35 questionnaires were completed at 3 time points: preoperatively, on day 7 and day 30 postoperatively. Internal consistency (Cronbach’s alpha), convergent validity (Spearman correlation with EORTC QLQ-H&N35), and responsiveness (Cohen’s d effect size) were evaluated. Results. Internal consistency was excellent for the global score (α=0.913), good for the emotional subscale (α=0.843), acceptable for the functional subscale (α=0.773), and borderline acceptable for the physical subscale (α=0.622). Convergent validity was confirmed by strong negative correlations with corresponding EORTC domains (ranging ρ=−0.493 — −0.708; p <0.0001). The questionnaire demonstrated high responsiveness to clinical changes with very large effect sizes both for deterioration in the early postoperative period (d ranging −2.511 — −4.075) and for improvement during recovery (d ranging +1.659 – +2.582). Conclusions. The Ukrainian version of MDADI is a valid and reliable instrument for assessing dysphagia-related quality of life in oropharyngeal cancer patients after surgical treatment.
